When it comes to your health, zip code is more important than genetic code.

The goal

Blue Cross Blue Shield Association (BCBSA) collectively insures one in three Americans. Their mission is to improve the health of members and communities across the United States. One of their key initiatives is the Community Health Management Hub aka CHM Hub, a real-time platform that highlights the social and environmental conditions of daily life that drive more than 60% of health outcomes.

The challenge

When Loka partnered with BCBSA, we discovered that CHM Hub was originally built on a legacy system and was outdated and hard to find. Without a modern means of collecting and analyzing community health data in real time, BCBSA struggled to understand behavioral and environmental patterns, make strategic decisions and improve health outcomes in hard-hit neighborhoods.

The solution

Loka conducted a thorough analysis of BCBA’s existing systems and processes to identify areas of improvement. With intimate knowledge of outdated building blocks and cutting-edge tools and workflows, we set out to update CHM Hub’s legacy system. The solution moved CHM Hub to a modern and enduring technical foundation that enabled healthcare providers to make data-driven decisions through new modules, including scenario planning, network calculations and guided reporting.

Highly specific analysis of impacted areas revealed new insights about America’s opioid crisis, suggesting a geographic relationship of diagnosed opioid use disorder along major highways and uncovering new means of prevention.

CHM Hub highlights nutrition deserts

The platform uncovered nutrition deserts and transportation deserts, two leading determinants of health, and brought them to the attention of private and public solutions providers.

CHM Hub helps organizations develop strategies to address gaps in resources, optimize product-market fit for specific geographies, preview the impact of strategy deployment, track progress and optimize strategy.

Automating 80% of data
